Regency at Chagrin Falls offers a full spectrum of post-hospital services, from a short-term rehabilitation stay to long-term nursing care.
Transitional Care
Regency at Chagrin Falls places great value on our ability to make a guest's transition through the health care system as comfortable and beneficial as possible. Transitional Care focuses on the coordination and continuity of healthcare from hospital to home.
In addition to direct care nurses, Ciena Healthcare utilizes dedicated and experienced RNs to oversee the intake of clinical information from the hospital, perform admission assessments, care plan development, and guest education to ensure a safe discharge home, and physicians provide in-house coverage at least five days per week to identify and respond to any clinical issues that could result in slowed progress or hospital readmission.
Also available are therapists (up to seven days per week) to maximize each guest's recovery. Prior to a guest going home, our team will perform a home assessment if appropriate and identify any equipment or modifications that would enhance the guest's recovery or safety. Guest and family education is geared toward the specific diagnoses of each individual. In addition, we will coordinate with the guest's home healthcare company and contact the family physician to make sure all follow-up care is scheduled.

Today’s SNFs have significantly evolved over time to provide both long-term comprehensive care and short-term rehabilitation services. However, in comparison to other medical settings, especially hospitals, SNFs are often overlooked for important resources and funding.
As health care evolves, skilled nursing communities are increasingly providing care previously delivered in hospitals. Nationwide, the 65 and older population will double over the next 20 years. By 2040, it could triple. As our population ages, SNFs are becoming increasingly vital to the care and wellbeing of our seniors.
