The best way for families to stay involved in a resident’s care plan is to visit. It’s easier to advocate for your loved one when you are there in person. If visiting is not an option, use multiple channels to keep in contact with the facility; email, phone, family portals, etc. If one person in the family is managing the care plan, share new information with other family members so no one is blindsided by care-plan changes.
